What is Apap Extra?

It is a medicine with analgesic and antipyretic effect, containing Paracetamol and Caffeine .

What does Apap Extra contain and how does it work?

It is a combined preparation containing paracetamol and caffeine. Paracetamol has analgesic and antipyretic effects. The mechanism of its action consists in inhibiting the activity of cyclooxygenase 2 (COX-2) in the brain and in the spinal cord, the enzyme responsible for the synthesis of prostaglandins. Paracetamol has no anti-inflammatory activity because it practically does not inhibit the activity of cyclooxygenases and prostaglandin synthesis in peripheral tissues. It does not damage the gastric mucosa. It does not inhibit platelet aggregation and does not affect the coagulation process. The second component of the preparation is caffeine. Caffeine enhances the analgesic effect of paracetamol. It also has a moderately stimulating effect on the central nervous system. After oral administration, paracetamol and caffeine are well and quickly absorbed from the gastrointestinal tract. Both components of the preparation penetrate the placenta and into breast milk.

When should I use Apap Extra?

The preparation is indicated in pains of various origins:
· Headaches, including MIG raine headaches
· Muscle pain and osteoarticular pain
· Toothache
· neuralgia
· Dysmenorrhea
· Post-traumatic and post-operative pain
· To relieve cold and flu symptoms such as fever, sore throat, muscle pain and osteoarticular pain.

When not to use this preparation?

Unfortunately, even if there are indications to use the preparation, it can not always be used. You can not use the preparation if you are allergic (you are hypersensitive) to any of the ingredients.
The use of the preparation is contraindicated in people:
with severe renal or hepatic impairment
with congenital deficiency of Glucose -6-phosphate dehydrogenase or methemoglobin reductase
with alcoholism
with heart rhythm disturbances
using MAO inhibitors in parallel or in the last two weeks,
in children up to 12 years old
in women in the first trimester of pregnancy and breastfeeding.

Take special care with Apap Extra?

Certain diseases and other circumstances may constitute a contraindication to use or indication to change the dosage of the preparation. In some situations it may be necessary to carry out specific check-ups.
Other paracetamol-containing medicines should not be used in parallel because of the risk of overdose and serious side effects. Check whether other medicines used do not contain paracetamol.
In people who are debilitated, starved, with liver failure or regularly consuming alcohol, there is an increased risk of liver damage.
Do not consume alcohol during the use period. Consumption of alcohol during the use of the preparation may lead to toxic liver damage and its inefficiency.
Caution should be exercised in patients with impaired renal or hepatic function, patients with bronchial asthma, hyperthyroidism, and in people with insomnia sleep disorders.

Does this preparation affect the ability to drive vehicles?
Sometimes the preparation may cause side effects such as, for example, inability to focus, tremor or dizziness, which may impair psychomotor performance.

Dosage of Apap Extra

The preparation is in the form of coated tablets for oral use. Use the preparation as recommended. Do not exceed the recommended doses because it will not increase the effectiveness of the drug and may harm your health and life. If you have any doubts regarding the use of the preparation, consult a physician.
Adults and adolescents after 12 years: 1-2 tablets every 4-6 hours. The maximum dose is 8 tablets a day.
Do not use in children under 12 years of age.

Can I use Apap Extra during pregnancy and breastfeeding?

During pregnancy, do not use any medicine without consulting your doctor!
It is very important that before using any medicine during pregnancy or breastfeeding, consult a physician and explain beyond any doubt the potential risks and benefits associated with the use of the drug. If you are pregnant or planning to become pregnant, consult your doctor before using the product.
The use of the preparation in women during the first trimester of pregnancy and breast-feeding is contraindicated. Before using the preparation in the second and third trimester of pregnancy, a doctor should be consulted.

Can I use other preparations in parallel?

Tell your doctor about all medicines you have recently taken, including those that are over the counter. In particular, consult your doctor if you are using any of the medicines listed below, as you may need to change your dosage or stop taking your medicine.
It must not be used if you are taking other medicines containing paracetamol (risk of overdosing with acetaminophen and experiencing severe side effects) or with caffeine.
The use of the preparation with MAO inhibitors is contraindicated and within 2 weeks after discontinuation of their use. Paracetamol used with MAO inhibitors may cause arousal and high fever.
Be careful and consult your doctor before taking the product if you are taking: oral antidiabetic agents, anticoagulants (eg Warfarin ), rifampicin, antiepileptic drugs, sedatives or hypnotics, zidovudine, sympathomimetic medicines (eg, nasal congestion drugs, appetite suppressants, amphetamine-like psychostimulants), thyroid hormones, oral contraceptives, cimetidine, chloramphenicol, quinolones or Verapamil .
Paracetamol may increase the effect of anticoagulants from the coumarin group (eg warfarin), increasing the risk of bleeding.
Do not use with zidovudine. Paracetamol may increase the toxic effects of zidovudine (risk of neutropenia, risk of liver damage).
Medicines increasing hepatic metabolism, including rifampicin, antiepileptics (eg phenytoin, carbamazepine), barbiturates (eg phenobarbital), hypnotics and alcohol used in parallel with paracetamol can lead to liver damage, even at the recommended doses.
Administration of paracetamol in combination with chloramphenicol may increase the concentration and toxicity of chloramphenicol.
The concomitant use of paracetamol with NSAIDs increases the risk of kidney damage.
Consumption of alcohol during the use of the preparation may lead to toxic liver damage and its inefficiency. Alcohol should not be consumed while using the product.

What side effects can occur?

Like all medicines, Apap Extra can also cause side effects, although they will not occur in all patients who use this medicine. Remember that the expected benefits of using the drug are usually greater than the damage resulting from the occurrence of side effects.
Due to the content of paracetamol, the preparation may cause: nausea, vomiting, digestive disorders, liver dysfunction (in case of paracetamol overdose acute liver failure, liver damage or necrosis may occur), renal papillary necrosis, renal colic, acute renal failure , hematological disorders ( thrombocytopenia, agranulocytosis). Possible hypersensitivity reactions: pruritus, urticaria, rash, erythema as well as life-threatening angioedema and anaphylactic shock (symptoms may include swelling of the larynx, tongue, throat, shortness of breath, bronchospasm, confluent sweating, increased heart rate, rapid drop in blood pressure, loss of consciousness). Due to the caffeine content, the preparation may cause: cardiac arrhythmias, insomnia, inability to focus, muscular tremors, irritability, dizziness, gastrointestinal disorders.
