What is Abelcet?
Antifungal medicine for general use.
What does Abelcet contain and how does it work?
The active substance is amphotericin B, an antibiotic with antifungal activity, produced by the strainStreptomyces nodosus. Amphotericin B is present in a lipid-bound form in this preparation. This form of antibiotic is less toxic than the conventional form of amphotericin B and can be used at higher doses. Amphotericin B is effective against many species of yeast, cryptococcus, mold and other pathogenic fungi. It has a high affinity for ergosterol, a fungal cell membrane component. Binding the antibiotic with ergosterol causes damage to the cell membrane, resulting in the death of the fungal cell. After intravenous administration, amphotericin B in bound form rapidly penetrates tissues and organs. Compared to the conventional form of amphotericin B, the antibiotic-bound form achieves higher concentrations in the liver, spleen, lungs and bone marrow. In the kidneys, a tool particularly exposed to the toxic effects of the antibiotic, it reaches concentrations several times smaller than that of amphotericin B in a conventional form. This contributes to a significant reduction of the nephrotoxicity of the preparation.
When to use Abelcet?
The preparation is indicated for the treatment of the following fungal infections:
· Severe invasive candidiasis
· As a second-line drug in severe systemic mycoses in patients who are unresponsive to amphotericin B in unbound form or other generally acting antifungals, in patients with renal insufficiency or other contraindications for use of amphotericin B in unbound form and in patients with kidney damage caused by amphotericin B
as a second-line drug in invasive aspergillosis, cryptococcal meningitis, disseminated cryptococcosis in HIV patients, fusaria, coccidioidomycosis, zygomycosis and blastomycosis.
When not to use this preparation?
Unfortunately, even if there are indications to use the preparation, it can not always be used. You can not use the preparation if you are allergic (you are hypersensitive) to any of the ingredients.
Take special care with Abelcet?
Certain diseases and other circumstances may constitute a contraindication to use or indication to change the dosage of the preparation. In some situations it may be necessary to carry out specific check-ups.
Do not use the preparation for the treatment of asymptomatic fungal infections, diagnosed only on the basis of skin tests or serological tests.
If you have ever had kidney problems or kidney problems, consult your doctor before using the product. If necessary, your doctor will advise you to check your kidney function before starting treatment, and then at least once a week in each treatment. It will also recommend regular testing of serum electrolytes, especially of potassium and Magnesium
Sometimes, in people with risk factors for liver dysfunction, the use of the preparation may be associated with the occurrence of increased bilirubin and with increased liver enzymes. These patients should be under strict medical supervision. If your liver gets worse, your doctor may advise you to stop using it.

Does this preparation affect the ability to drive vehicles?
It is unlikely that the preparation will impair the ability to drive or use machines / machines.
Dosage of Abelcet
The preparation is in the form of a concentrate for the preparation of a suspension of liposomes for intravenous infusion. The doctor will choose the appropriate dosage depending on the severity and type of infection. The drug is administered intravenously by qualified medical personnel, usually in a hospital. If you have any doubts regarding the use of the preparation, consult a physician. In certain special patient groups, additional dosage adjustment is necessary.
Normally 5 mg / kg body weight per day for at least 14 days. The product is administered intravenously as a drip infusion at a rate of 2.5 mg / kg body weight per hour.
The preparation can be used by people with diabetes.
In systemic mycoses in children, aged 1 month-16 years, the doctor will recommend the use of appropriate doses depending on the child's body weight (doses per kg of body weight are comparable to those recommended in adults).
Special groups of patients:
Elderly patients do not need to adjust their dosage.
In patients with impaired renal or hepatic function there is no need to change the dosage. In patients with impaired renal function, caution and regular renal function tests are necessary.
The preparation can be used in the treatment of systemic mycoses in patients with severe neutropenia (neutrophil deficiency).
Method of administration:
The preparation can only be given as an infusion into a vein. Before serving, it must be properly diluted. If the preparation is used for the first time, the infusion dose will be infused over the first infusion for approximately 15 minutes. After dosing the patient will remain under close observation for 30 minutes. If the symptoms of hypersensitivity do not occur, the infusion will continue.
Can Abelcet be used during pregnancy and breastfeeding?
During pregnancy, do not use any medicine without consulting your doctor!
It is very important that before using any medicine during pregnancy or breastfeeding, consult a physician and explain beyond any doubt the potential risks and benefits associated with the use of the drug. If you are pregnant or plan to become pregnant, please inform the doctor prescribing the prescription for this medicine.
Do not use the preparation in pregnant women, except in life-threatening cases, when in the doctor's opinion the benefits for the mother outweigh the danger to the fetus. Only a doctor can assess the benefit / risk ratio in your case.
You should not breastfeed during treatment.
Can I use other preparations in parallel?
Tell your doctor about all medicines you have recently taken, including those that are over the counter.
Amphotericin B in the liposome suspension may have nephrotoxic effects (toxic to the kidneys). If other medicines with potential nephrotoxicity are used in parallel, special care and close monitoring of renal function is necessary.
If it is necessary to use the preparation and zidovudine concurrently, the kidneys and hematopoietic system should be carefully monitored (risk of increased nephrotoxicity and myelotoxicity).
Parallel ciclosporin may cause an increase in blood creatinine.
There are no data on specific drug interactions with other drugs. It is known that amphotericin B in an unrelated form to liposomes can interact with anticancer drugs, corticosteroid
s and corticotropin (ACTH), digitalis glycosides and skeletal muscle relaxants.
Do not transfuse white blood cells (leukocytes) in people using amphotericin B (risk of acute toxic effects on the lungs).
What side effects can occur?
Like all medicines, Abelcet can also cause side effects, although it will not occur in all patients who use this medicine. Remember that the expected benefits of using the drug are usually greater than the damage resulting from the occurrence of side effects.
Very often you may experience: increased creatinine
, chills, fever.
Common: alkaline phosphatase, increased blood urea, tachycardia, arrhythmias, thrombocytopenia, headache, tremor, shortness of breath, asthma attack, nausea, vomiting, abdominal pain, renal dysfunction, renal insufficiency, rash, hyperbilirubinaemia , hypokalaemia, electrolyte imbalance, including increased potassium in the blood, decreased magnesium levels in the blood, hypertension, hypotension, abnormal liver function tests.
Uncommon: cardiac arrest, convulsions, neuropathy, respiratory failure, muscle pain, pruritus, injection site reactions, anaphylactic reaction.
In addition, with unknown frequency, encephalopathy, bronchospasm, hypostenuria (no kidney ability to produce adequately concentrated urine), tuberculosis, exfoliative dermatitis, shock may occur.
If amphotericin B is administered as a lipid complex, side effects associated with amphotericin B in an unbound form can not be ruled out. Due to the lower nephrotoxicity of the preparation than in the case of the conventional (unrelated to lipids) form of amphotericin B, decreased deterioration of renal function was less frequently observed and no significant nephrotoxicity was observed.
