What is Adamon SR?

Opioid analgesic drug containing tramadol.

What does Adamon SR contain and how does it work?

Tramadol is a powerful narcotic analgesic from the opioid group, stimulating mainly receptors called μ receptors (less stimulates receptors)kappa anddelta). The strength of its action is determined as 1/6 to 1/10 of the potency of morphine.
Another mechanism of action of tramadol consists in influencing the communication between norepinephrine and serotonin-dependent nerve cells. The place of connection of two neurons is called the synapse. The cell that transmits the information in front of the synapse releases a so-called mediator into the synaptic cleft, a substance that is captured and recognized by the cell receiving information behind the synapse. Some of the mediator molecules are captured back by the neuron receptors in front of the synapse, which we call reuptake. The action of tramadol consists in inhibiting the process of norepinephrine and serotonin reuptake and, consequently, prolonging its duration in the synapse and stimulation of the recipient cell. The result is a modification of the transmission of pain impulses.
Tramadol has antitussive activity, in contrast to morphine at the recommended doses does not cause respiratory depression nor does it disturb gastrointestinal motility. It has only a small impact on the cardiovascular system.

When should Adamon SR be used?

The product is indicated for the treatment of moderate to severe pain.

When not to use this preparation?

Unfortunately, even if there are indications to use the preparation, it can not always be used. You can not use the product if you are allergic (you are hypersensitive) to any of the ingredients or other opioid analgesics.

Contraindications to the use of the preparation are also:
· Acute alcohol intoxication, hypnotics, centrally acting analgesics including opioids or psychotropic drugs
· Parallel use of MAO inhibitory drugs (tramadol should not be used during and 14 days after the end of therapy with MAO inhibitors )
· Treatment of opioid addiction.

When should you use extreme caution when using Adamon SR?

Certain diseases and other circumstances may constitute a contraindication to use or indication to change the dosage of the preparation. In some situations it may be necessary to carry out specific check-ups.

Tramadol is a drug with low addictive potential. However, in the case of long-term use of the preparation, tolerance may occur (the need to administer increasing doses of the drug to achieve the same therapeutic effect) and mental and physical dependence.

In people with a tendency to abuse drugs and addiction, the doctor will probably recommend taking breaks in taking the drug. During treatment, the patient should remain under close medical supervision.

If no higher than recommended doses are needed during treatment, withdrawal symptoms are rare.

The preparation should not be used in people addicted to opioids, it does not reduce the symptoms of discontinuation of morphine.

In some cases, convulsions have been reported with the recommended doses of tramadol (the risk increases with high doses above 400 mg / day). People with epilepsy or those prone to convulsions should be treated with tramadol only if absolutely necessary. The risk of seizures may increase in people who use other medicines at the same time to lower the seizure threshold.

The preparation should be used with caution in people:
· With consciousness disorders of unknown etiology
· With severe liver and / or kidney failure
· With related conditions
· With increased intracranial pressure (head injuries, brain disease)
· With disorders of respiratory center and respiratory function.

At the recommended therapeutic doses, the preparation does not cause breathing disorders. However, caution should be exercised in people with impaired breathing or excessive bronchial secretions in the airways, and also in parallel with drugs that inhibit the activity of the central nervous system because in these cases the preparation may cause breath suppression.

The preparation contains sucrose; should not be used in people with rare hereditary galactose intolerance, primary lactase deficiency or malabsorption of Glucose -galactose.
Opioid analgesics impair the mental and / or physical fitness required to perform potentially dangerous tasks (eg driving or operating machines). Do not drive or use machines while using the product.

Dosage of Adamon SR

The preparation is in the form of prolonged-release capsules. Do not exceed the recommended doses because it will not increase the effectiveness of the drug and may harm your health and life. If you have any doubts regarding the use of the preparation, consult a physician.
Adults and children over 12:
Initially, 50-100 mg 2 times a day is used. If necessary, your doctor may decide to increase the dose to 150-200 mg 2 times a day (up to a maximum of 400 mg a day).
In patients over 75 and patients with moderate renal or hepatic insufficiency, your doctor will probably recommend an appropriate dose interval.
Do not use the preparation in people with severe liver and / or kidney failure.

The parallel use of tramadol with drugs that affect the central nervous system or alcohol may cause mutual aggravation.

Very rarely, convulsions have been reported after the parallel use of tramadol with drugs that lower the seizure threshold or cause convulsions (eg medicines used to treat certain mental disorders).

The concomitant use of carbamazepine causes a significant reduction in the concentration of tramadol in the blood, which may reduce the effectiveness and reduce the duration of analgesia.

Parallel administration of ritonavir may result in increased tramadol blood concentrations and consequent onset of its toxic effects.

During the parallel administration of Digoxin and tramadol, side effects of digoxin have been reported rarely.

CYP3A4 cytochrome P450 isoenzyme blockers (eg ketoconazole, Erythromycin ) can inhibit the metabolism of tramadol, which leads to an increase in its concentration in the blood.

You should not drink alcohol during treatment.

What side effects can occur?

Like all medicines, Adamon SR can cause side effects, although they will not occur in all patients who use this medicine. Remember that the expected benefits of using the drug are usually greater than the damage resulting from the occurrence of side effects.
Long-term use of the preparation may lead to addiction. The following withdrawal symptoms may occur: agitation, anxiety, nervousness, insomnia, hyperkinesia, muscular tremors, and stomach and intestinal disorders.

Very common: dizziness, nausea.
Common: headaches, drowsiness, vomiting, constipation, dry mouth, sweating.
Uncommon: cardiac arrhythmias, tachycardia (acceleration of the heart rate), circulatory collapse, orthostatic hypotension, retching, gastrointestinal discomfort (feeling of pressure around the stomach, feeling of fullness), skin reactions (eg pruritus, rash, urticaria).
Rarely: changes in appetite, mental disorders of varying severity and character (depending on the patient's personality and duration of treatment). These include changes in mood (mainly elevation of mood, sporadic dysphoria), changes in activity (usually limiting, occasionally increasing), and changes in cognitive and sensory functions (decision making, perception disorders), blurred vision, bradycardia (slowing the heart rhythm), elevation of blood pressure , micturition disorders (disturbances in urination, urinary retention).
Very rare: allergic reactions (eg dyspnea, bronchospasm, wheezing, edema).
In isolated cases, elevated liver enzymes have been reported during treatment with tramadol.
